Duties
The social workers for this announcement are assigned to Social Work Services and work is performed on the campus of the VA Long Beach Healthcare System (VALB), Long Beach, CA. The multiple positions recruiting are assigned to various inpatient and outpatient programs to include Inpatient Medicine Service; Outpatient Extended Care Programs, Outpatient PACT-Primary Care and Inpt Psychiatry Unit. The incumbent is an independent licensed social worker (GS-11) and/or registered associate social worker (ASW) with the California Board of Behavioral Sciences. Current social work practice skills are demonstrated by: clinical experience in acute/complex healthcare settings; utilizes shared decisions making skills with patients in developing individualized admission assessments, discharge planning and or coordination of care plan; knowledge, skill and/or experience working with adults who have a variety of acute or complex medical, emotional and/or resource issues; knowledge and/or skill is the assessment of domestic and intimate partner violence, all forms of abuse and CA mandatory reporting requirements; knowledge and/or experience working as a part of an inpt or outpt multidisciplinary team; demonstrates ability to be proactive in communicating between the patient/family and the multidisciplinary team; ability to work in fast-paced clinical hospital/healthcare settings.
· Inpatient Medical/Surgical units- specialized population: adults Inpt acute and general medicine, intensive care and surgical settings; care provided to those with multiple medical conditions; proactively assesses discharge to home or community programs and responsible to assure potential barriers affecting discharge are addressed; adapt clinical social work skills to effectively meet functions assigned in a high-paced setting with multiple admission and discharges.
· Inpatient Acute Psychiatry- specialized population: adults Inpt Acute and Gero psychiatric settings; care provided to those with multiple acute mental health conditions; work with LPS conservatorships; use community mental health approach; coordinate discharges to home or community programs; adapt clinical social work skills to effectively meet functions assigned in a high-paced inpatient setting with multiple admission and discharges.
· Geriatric Extended Care Programs - Community Living Center, Community Nursing Home, Community Residential Care - specialized population: adults Inpatient and outpatient geriatrics; acute and short stay rehabilitation; psycho-educational support, transitioning care and end of life planning; assist Veterans/Caregivers conservatorship, ombudsman, follow-up visits community Nursing Homes; adapt clinical social work skills to effectively meet functions assigned geriatric and rehabilitative care, family/Caregiver report, death and end of life planning and support.
· Patient Care Aligned Teams (PACT) – outpatient Primary Care teamlet(s) – Specialized adult population: multi-medical conditions and psychosocial, function as a multidisciplinary team; provide various levels of care management, assess and provides psychosocial resources.
VALB serves as a medical and mental healthcare training facility and social workers work with other disciplines that rotate as a part their training program; demonstrate skills a providing education of advanced care planning; demonstrate skills documenting clinical care using an electronic medical record system; ability to provide care in the non-traditional office setting-such as shared work space; clinical care may take place, bed side, medical units hallways, group rooms, and/or over the phone and the social worker is often mobile, moving from varying units, floors or buildings.
This medical center operates 24 hours, seven days per week and you may be required to work days, evenings, nights, holidays, and/or weekends, to include modification in shifts and/or tours of duty, to meet medical center and patient care needs.
Per VHA Directive, all qualified social workers are responsible to provide after-hours on-call to the Emergency Department. Qualified social workers may volunteer for a calendar year on-call rotation schedule. In the event there are not enough volunteers, all qualified social workers will be assigned and rotated to the calendar year on-call schedule by the Chief, Social Work Svcs. Compensation is provided when after hour's on-call.
