Duties


This position is located in Community Engagement and Reintegration Service (CERS) at the Greater Los Angeles Healthcare System, Los Angeles, California.  As a Social Worker for Housing and Urban Development – VA Supportive Housing (HUD-VASH), you will be responsible for providing case management for Veterans in the HUD-VASH program, providing appropriate treatment and maintaining Veterans in appropriate housing.   You will be responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements.   Major duties and responsibilities include, but are not limited to, the following:

·         Performs site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes;

·         Verifies Veteran's status, eligibility for VA medical care and eligibility of program participation and ensures consideration as primary program participants;

·         Coordinates care and serves as an advocate on behalf of Veterans to ensure comprehensive service delivery;

·         Participates in program planning and development and advises of trends with caseloads, identifies needs for changes and makes recommendations;

·         Conducts psychosocial assessments to determine needs of Veterans and family members;

·         Participates in the treatment planning process with other professionals of differing disciplines;

·         Provides individual, family and group counseling as appropriate; and

·         Provides the full range of social work services within commonly accepted standards of social work practice which includes case work, individual and group counseling, discharge planning, case management and other services as appropriate.

Work Schedule: Full-Time, Monday - Friday, 8 am - 4:30 pm
