The VA Greater Los Angeles Healthcare System (VAGLAHS) is one of the largest health care facilities within the Department of Veterans Affairs. It is one component of the VA Desert Pacific Healthcare Network (VISN22) offering services to Veterans residing in Southern California and Southern Nevada. VAGLAHS consists of two ambulatory care centers, a tertiary care facility and 8 community based outpatient clinics. VAGLAHS serves Veterans residing throughout five counties: Los Angeles, Ventura, Kern, Santa Barbara, and San Luis Obispo. There are 1.4 million Veterans in the VAGLAHS service area. VAGLAHS is affiliated with both UCLA School of Medicine and USC School of Medicine, as well as more than 45 colleges, universities and vocational schools in 17 different medical, nursing, paramedical and administrative programs.

Duties

This position is located with the Greater Los Angeles VA Healthcare System in Los Angeles, CA for an Advance Practice Social Worker who will provide services to Veterans in Primary Care on the Patient-Aligned Care Team (PACT). You will function as a member of the comprehensive interdisciplinary team that delivers primary care to Veterans in a longitudinal fashion. Duties of this position include, but are not limited to:
·   Conducts comprehensive assessments of Veterans and develops treatment plans that are sensitive to Veterans diverse needs and abilities;
·   Independently performs a full range of clinical services including Individual, Group and Family interventions;
·   Provides on and off site crisis intervention and outreach (individual sessions, assist and coordinate hospitalizations in conjunction with the physician when required, coordination of treatment with collaterals (family, housing programs, nursing homes, mental health, etc.) to maximize the successful stabilization and integration of Veterans in the community both in crisis and non-crisis situations;
·   Independently provides a full range of concrete services including completing various forms and applications to insure Veterans receive all benefits and services they are entitled to (i.e. nursing home placement, board and care, SSDI, and State Veterans Homes, etc.);
·   Provides assistance in the procurement and retention of benefits, housing and where specialized services are required (i.e. legal issues, home care, shelter evaluation and referrals, financial, transportation, food, Polysubstance abuse, patient education) and will assist the Veteran in learning about and linking to the appropriate resources they require;
·   Provides consultation to colleagues on the psychosocial treatment of patients in specialty areas, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills;
·   Supports team members with challenging patient-care situations;
·   Utilizes principles of human growth and development over the life span and will be able to assist Veterans in coping with the loss and grief experiences from disability and terminal illness;
·   Serves on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor and Chief of Social Work Service;
·   Furthers own professional growth through continuing education and for ensuring continuing education appropriate to the area assignment;
·   Provides assistance with the orientation of inexperienced social workers and with the permission of the supervisor and 2 years of VA experience, may serve as a field instructor for social work trainees;
·   Uses software applications for drafting documents and data management as well as other computer systems in use by the VHA; and
·   Maintains a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work standards, Joint Commission standards, and facility standards.
Work Schedule: Monday through Friday, 8am- 4:30pm.  You may be required to assist in providing Social Work coverage in the Emergency Department during weekends, weeknights, and holidays, as assigned.

Qualifications

Basic Requirements: The basic requirements for employment as a VHA Social Worker are prescribed by statute in 38 U.S.C. 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000.
U.S. Citizenship: Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Must possess a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may notbe substituted for the master's degree in social work. (Transcripts required).
Licensure: Must be licensed or certified by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced level (except for licenses issued in California, which administers its own clinical examination for advanced practice). May have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social).  Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment.  You will receive credit for all qualifying experience, including volunteer experience.
GRADE DETERMINATIONS
GS-12 Social Worker- Experience: At this level you should have experience that demonstrates possession of advanced practice skills and judgment and with at least 2 years of experience in an area of specialized social work practice. Advanced practice is the ability to provide independent and expert clinical psychosocial and case management services in a specialized area of practice to people who tend to have serious and severe crises, may lack any familial and community support, may be poor self-monitors, may frequently fail to comply with instructions and treatment, or may have major deficits in coping skills and require continuing professional psychological support.
In addition, the candidates must demonstrate the KSAs listed below.
Demonstrated Knowledge, Skills, and Abilities:
1. Advanced knowledge of and mastery of theories and modalities used in the specialized treatment of complex physical or mental illness. Ability to incorporate complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Ability to determine priority for services and provide specialized treatment services.
2. Advanced and expert skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management interventions used in the treatment of veterans with polytrauma injuries, spinal cord injuries, traumatic brain injuries, visual impairment, post-traumatic stress disorder, etc.
3. Advanced knowledge and expert skill in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services. Ability to coordinate the delivery of specialized psychosocial services and programs. Ability to design system changes based on empirical findings.
4. Ability to provide subject matter consultation to colleagues and students on the psychosocial treatment of patients treated in the specialty area, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills. Ability to teach and mentor staff and students in the specialty area of practice and to provide supervision for licensure or for specialty certifications.
5. Ability to expand clinical knowledge in the profession, demonstrating innovation in the creation of new models of psychosocial assessment or intervention to identify and address specialized clinical needs. Ability to write policies, procedures, and/or practice guidelines pertaining to the specialty population or specialty treatment program.
